  • IMPORTANT NOTE: CHHS does NOT fax, email, or mail any student records to any colleges or universities. It is the responsibility of the student to get their records to the college or university. 

     

    IMPORTANT NOTE ABOUT TRANSCRIPTS DURING THE SUMMER: 

    Counselors are not available over the summer to upload electronic transcript requests (this includes Common App and SendEDU). Students may still request transcripts through GA Futures. Have a safe and restful summer!

    What to do BEFORE scheduling an appointment or attending a walk-in session: 

    1. Complete the district's online registration, which includes uploading required registration documents: GCPS Online Registration. Instructions on how to upload required registration documents can be found here
    2. After submitting the online registration and the required documents, please contact the counseling office at 770.682.4106 if scheduling an appointment for June dates. For walk-in sessions, please arrive at the school between 7:30 AM - 10:30 AM. 

    ** Incomplete paperwork will delay your registration. We MUST receive ALL required registration documents in order to proceed with the appointment or walk-in session. **

    What to BRING to the appointment or walk-in session (if not uploading through the online registration): 

    • Withdrawal paperwork from former school
      • If student is a Rising 10th, 11th, or 12 grader, paperwork must include transcript and discipline record
      • If student is a Rising 9th grader, paperwork must include the final 8th grade report card with proof of promotion to the 9th grade. 
    • Student's GA 3231 Immunication form
    • Student's Birth Certificate
    • Photo ID of enrolling parent/guardian
      • Legal guardian must have official documentation from Probate Court
    • Proof of residency which MUST include the following two documents: 
      • Current Mortgage Statement OR Property Tax Bill OR Lease Agreement
      • One of the following utility bills: 
        • Water
        • Natural Gas
        • Electricity
      • NOTE IF LIVING WITH ANOTHER FAMILY: Per Gwinnett County Public Schools Guidelines: Students who live with their parents or legal guardians but reside in the home of another adult and are seeking enrollment into Gwinnett County Public Schools must complete a Residency Affidavit form and turn it into your school upon registration.     What to expect at your appointment: 

    A CHHS counselor will provide academic advisement for the 2021-2022 school year. First semester core classes and electives will be selected at this time. Please allow up to one hour if coming to a scheduled appointment and possibly longer if attending a walk-in session. 

    NOTE: Please see Summer 2021 Reading and AP Assignments for details regarding summer reading and AP Assignments. 

  • Registration: Students New to GCPS

    ***Only for students that have NEVER attended a Gwinnett County Public School.  If your student has attended a GCPS school in the past, please refer to the instructions for students previously enrolled in GCPS.​​​***
      
    1. Complete the district's online registration process which includes uploading required registration documents: GCPS Online Registration. Instructions on how to upload required registration documents can be found here.
    2. After submitting the online registration and necessary documents, please contact the counseling office at 770-682-4106 to schedule an appointment.

    Documents Required for Enrollment:

    • TWO proofs of residency (lease/mortgage statement AND a current gas, water, or electric bill)
    • Student's birth certificate
    • Enrolling parent/guardian's driver’s license or other government issued photo ID
    • Student's Social Security card
    • Current immunization record (Georgia Form 3231)
    • Withdrawal packet from the previous school (including discipline record and current transcript/course history)
    • Copy of student's current IEP, if applicable (applies to students in Special Education)

    **Please note that incomplete paperwork will delay your registration and your student's start date.

  • Registration: Students Transferring from Another GCPS School

    1. To transfer schools within GCPS, you must first withdraw your student from their current school.  
    2. Log on to your existing Parent Portal account, click on ParentVue >> Online Registration >> Begin New Registration.  Once you have completed the registration and have uploaded the required registration documents, please call the counseling office at 770-682-4106 to schedule an appointment.

    Documents Required for Enrollment (please upload these with your online registration):
    • TWO proofs of residency (lease/mortgage statement AND a current gas, water, or electric bill)
    • Enrolling parent/guardian's driver’s license or other government issued photo ID
      • Note:  If the enrolling parent/guardian is different from initial GCPS enrollment form, we will also need proof of authorized person to enroll.  Please refer to the list of Required Registration Documents for details or call our office at 770-682-4106.

  • Registration: Students Previously Enrolled in a GCPS School

    Students who have previously attended a GCPS school at any point in their academic career (no matter the amount of time) already have a GCPS student ID number and are considered re-enrolling students. 

    1. Complete the district's online registration process: GCPS Online Registration. MAKE SURE to choose the option Registering Parent Guardian - NOT NEW to GCPS.  Your Username is the email address you used when last enrolled with GCPS.  If you have forgotten your password, please select 'Forgot Password' to reset it.  If you are unable to log in to your existing account, please contact the counseling office at 770-682-4106 or email jessica.rodriguez@gcpsk12.org for assistance.  Please do not set up a new account as this will create duplicate accounts and delay your registration process.
    2. Once you have completed the online registration,please call the counseling office at 770-682-4106 to schedule an appointment.

    Documents Required for Enrollment:
    • TWO proofs of residency (lease/mortgage statement AND a current gas, water, or electric bill)
    • Student's birth certificate
    • Enrolling parent/guardian's driver’s license or other government issued photo ID
    • Student's Social Security card

  • Transcript Requests

    A note about electronic transcript requests during Summer Break: Counselors are not available over the summer to upload electronic transcript requests (this includes Common App and SendEdu). Students may still request transcripts through GA Futures. Have a restful and safe summer! 

    Transcript requests options for CURRENT students: 

    • FREE: Students may send transcripts to ANY College/University in Georgia for FREE through Georgia Futures. Students will need to create an account with Georgia Futures and have a Social Security Number on file with CHHS. Click here to access Georgia Futures
    • To request a paper transcript, or to send an electronic transcript using an app other than GA Futures, please visit your My Payments Plus account to submit a transcript request and to submit payment. Please allow two business days for paper transcripts to be processed. Processing time on electronic transcripts varies and may take longer.

    Transcript Requests for Former Students of CHHS (2019-2020 & 2020-2021 school years):

    Please mail or bring the transcript request form, payment, and a copy of your photo ID, and your payment to:

    Payment must either be check or exact cash amount (we are not set-up to accept credit card payments in the office, and change is not available).

    PLEASE NOTE: Please allow 2 business days for transcripts to be processed.  


    Transcript requests for Students Who Last Attended CHHS in 2018-19 or Prior:

    Collins Hill High School no longer has these records on file. you will need to visit the Gwinnett County Public School Records Office website to request your transcripts.


    Note Regarding Social Security Numbers

    If you are attempting to submit a transcript through Georgia Futures and are receiving an error, it is likely that we do not have your Social Security Number on file. You will need to bring a copy of your Social Security card to the Counseling Office.

  • Withdrawals

    We wish your child the best during the transition to a new school. Please follow the steps below.  DO NOT USE the SUBMIT button.

    • Click on the link below and download/save the form to your device
    • Open the downloaded form with an Adobe application
    • Fill out the form
    • Convert the form into a PDF and send both the form and a copy of your picture ID to Ms. Wanda Bailey at wanda.bailey@gcpsk12.org

  • Address Change Requests

    ***PARENTS ARE NOW ABLE TO UPDATE CERTAIN INFORMATION VIA THE PARENT PORTAL***

    Address Changes:

    If you have moved, please update your student's address as soon as possible to ensure receipt of school mailings and to remain eligible for school transportation. We are required to have updated proofs of residency on file for all students (including digital learners).

    Please follow the steps below to update your student’s information:

    1. Fill out the Address Change Form.
    2. Email the form and proofs of residency to: Jessica.Rodriguez@gcpsk12.org

    NOTE: Please allow a minimum of two (2) business days for your requests to be processed. Students WILL NOT be issued a permanent bus change until ALL required documents are received.


    Emergency Contacts, Phone Numbers, and Email Addresses (only by Enrolling Parent):

    To update Emergency Contact information and/or parent phone numbers:

    1. Log on to your Parent Portal account
    2. Click on ParentVue >> Student Information >> Edit Information
    3. Follow the prompts to update each section as needed for each student

    To update the parent(s) email address: 

    1. Log onto your Parent Portal account
    2. Click on ParentVue >> Online Registration >> Begin New Registration
    3. Select the Parent/Guardian tab >> Edit (follow the Save and Continue prompts to complete updates)
    4. Review and submit
