  • Step 1:

    Register online, starting Monday, April 10, 2023

    Be sure to upload as many documents as possible to the website.  You will need the following to register:

    • Birth Certificate
    • Immunization Form 3231 from your local pediatrician or county health center
    • Hearing, Vision, Nutrition and Dental Information on Georgia Form 3300
    • Utility Bill
    • Proof of residency (lease, closing documents, tax bill)
    • Parent’s Photo ID
    • Child’s Social Security card
