GRS Beneficiary Direct Deposit Authorization Form
As a beneficiary, this form allows you to have your beneficiary benefits deposited directly into your checking, savings, or money market account each month. Please be sure to send the completed form to the attention of the Gwinnett Retirement Services Department at Gwinnett County Public Schools.
