Congratulations on being selected as a Support + Enrichment + Achievement (SEA) Tutor in GCPS! Please print and complete the following forms prior to your visit to the J. Alvin Wilbanks Instructional Support Center for your informational session and fingerprint completion.
- Federal W-4 Tax Withholding Allowance Form
- Georgia G-4 Tax Withholding Allowance Form
- Form I-9 Employment Eligibility Verification Form - You will only need to print pages 1 and 2. You must bring the original unexpired document(s) selected from the Lists of Acceptable Documents on page 3 for the I-9 form.
- Direct Deposit Authorization Form- A voided check or a direct deposit letter from your bank must be provided with the form.
