    IF LIVING WITH ANOTHER FAMILY: 

    Per Gwinnett County Public Schools Guidelines: Students who do not live with their parents or legal guardians but reside in the home of another adult and are seeking enrollment into Gwinnett County Public Schools must complete a Residency Affidavit form and turn it into your school upon registration.

  • Registration: Students New to GCPS

    1. Collect Click Link -> 2023-2024 Required Documents & New Student Registration Guide  (translations available soon via same link)

    2. Complete GCPS's Online Registration HERE  (instructions on how to upload documents HERE)

    3. Call 770-338-4870 to schedule an appointment. *Before an appointment can be scheduled, all required documents will need to be uploaded during the online registration process or dropped off at Central Gwinnett High School.*

    4. A current "Georgia 3231" Immunization Certificate is required. For more information, see Immunization tab below. 

      

  • Registration: Students Previously Enrolled in a GCPS School

    1. Collect  2023-2024 Required Documents  (translation available soon via same link)

    2. For previous GCPS students, complete the Online Regristration found in the Parent Portal (upper right corner). (Directions to upload documents are here.) 

    3. Login to your Parent Portal Account with the SAME email you used to create your account. (If you know the email but not the password, you can use the "forgot password" option in Parent Portal)

    If you FORGOT the email address you previously gave GCPS or your password, Do NOT create another Parent Portal/OLR account!

    If you know your email, but not password, use the "forgot password" button on the login screen.

    If you forgot your EMAIL address you used to create the account, please call the school so we can assist you or have the county assist if necessary.**

    4. Call 770-338-4870 to schedule an appointment. *Before an appointment can be scheduled, all required documents will need to be uploaded during the online registration process or dropped off at Central Gwinnett High School. 

    5. A current "Georgia 3231" Immunization Certificate is required. For more information on Immunization requirements, see the Immunization tab below.

     


  • Immunization

    By state law, a valid certificate of immunization (Georgia Form #3231) or a notarized statement of religious exemption (Form #2208) is required for enrollment. The certificate of immunization must be completed by the health department or your private physician. A valid Form #3231 must be marked with either "Date of Expiration" or as "Complete for School Attendance." (A certificate marked with a "Date of Expiration" expires on the date indicated. A current certificate must be submitted within 30 days of expiration.) A medical exemption, if applicable, should be noted on Form #3231 with a current date of expiration. You may request a form from your school for religious exemption.

    Please review immunization requirements for students entering 7th grade and a TDAP-Meningitis parent fact sheet. The new vaccine requirements apply to students who are born on or after 1/1/2002 AND entering 7th grade. The requirements also apply to students who are new entrants to 8th-12th grade. New entrants are students who have never attended any school in Georgia or have not attended any school in Georgia for at least one full school year.

    Before starting the 2021-22 school year, all students entering or transferring into 11th grade will need proof that they received a meningococcal booster vaccine (MCV4) on or after their 16th birthday.

    • MCV4 vaccines must be documented on Georgia Form 3231 Certificate of Immunization.
    • Students who are not 16 years old do not have to get the vaccine until their 16th birthday.
    • Note that this booster vaccine is required in addition to the MCV4 vaccine that was required for your child prior to his or her beginning 7th grade.

    Students can get vaccinations and certificates of immunization from their private physician or at any of the local health department offices.

  • Parent Portal Application

    Fill out the Parent Portal Registration Form (Bosanski/Srpski/Hrvatski中文;한국어 ;Español; Tiếng Việt)

  • Change of Address Form

    If you have moved, please fill out THIS Address Change FORM.

    Note: This form MUST be accompanied with all the required documents listed below:

    1.    Lease or Mortgage (including signature page)
    2.    Utility Bill (gas, water or electric)
    3.    Enrolling Parent/Guardian ID
    4.    *Residency Affidavit and THEIR ID (*only if your family is living with another family)

  • Withdrawals

    We wish your child the best during the transition to a new school. Our school system has an easy-to-use, secure online withdrawal process that will allow you to complete the form required for withdrawing your child.

    Once you complete and submit the online withdrawal form, you will be contacted by a school representative to verify your id and to provide you with any additional information necessary to complete the withdrawal if needed for your child.

    NOTE: All chromebooks and JROTC uniforms must be returned to CGHS to complete withdrawal. 

    Withdrawal Process (using any browser):

    1. Download THIS Withdrawal Form 

    2. Open the downloaded form with an Adobe application

    2. Fill out the editable fields with the information.

    3. Click the "Submit to GCPS" button (this will: open your email account, automatically attach your filled-out form, and send it to the correct people at CGHS)
