2022 Player Information Form 

Mill Creek High School has implemented a Card & Mobile Payment Only ticketing policy at all athletic events. Fans will be able to purchase tickets online through Mill Creek’s GoFan page, the GoFan app or by presenting a debit/credit card (or mobile payment option) at the ticket gate.

The new ticketing policy allows Mill Creek to operate a more efficient and professional ticket gate, while creating an enhanced fan experience. Purchasing tickets online, allows fans to easily access and share tickets (email or text) without standing in line.

A new ticket gate setup will help promote social distancing, while limiting person-to-person contact and the physical exchange of cash.

Mill Creek HS is now using Rank One for all pre-participation and physical forms.

Parents are required to have an account set up and all forms completed for their athlete prior to participating in any athletic activity at the school.

The Mill Creek Lady Hawks Softball Score Board is located behind the left center field wall at our softball facility. The score board can be seen from our softball stands, baseball stands, and the entry to Osborne Middle School.  Along with our scheduled home games we have daily visibility exceeding over 4000 students along with the traffic volume of parents and visitors to our schools and many other after-schools scheduled events.  Your business name and message will be shared with thousands of existing and potential customers on an annual basis as Mill Creek High School hosts events year round. Configurations that are available will be based on space at the beginning of our softball season and will be honored by contract length and years of participation on the score board.  Waiting lists will be built each year and open spots will be offered to our waiting lists first and then new companies.
