GCPS Transition Resources for Students with Disabilities

Welcome to GCPS Transition Resources!​

The goal of this site is to connect youth and young adults with disabilities to resources that can be helpful now and in preparation for life after high school. Planning and preparing for a successful transition to life after school is an ongoing process. It is never too early to start investigating. ​
