GCPS Transition Resources for Students with Disabilities

Welcome to GCPS Transition Resources!

The goal of this site is to connect youth and young adults with disabilities to resources that can be helpful now and in preparation for life after high school. Planning and preparing for a successful transition to life after school is an ongoing process. It is never too early to start investigating.

The agencies and information shared on this site reflect participants from the most recent GCPS Transition Fair. Please note that Gwinnett County Public Schools does not endorse the agencies posted on this website nor does it maintain associated agency website links. Questions?
