    State law forbids students from carrying any form of medication on them, including prescription medicine and all over-the-counter medicine such as Tylenol, Advil or cold/allergy medication. If it is necessary for a student to take medication at school, the medicine must be kept in the clinic and the parent must fill out the medication administration form.

  • Student Clinic Card

    Student Clinic Card - Clinic Cards must be updated each year along with any emergency contacts that are authorized to check out your student.

  • Administration of Medication Request

    Administration of Medication Request - Administration of Medication form must be filled out for each medication administered to the student. 

  • Allergy Plan

    Allergy Plan - An Allergy Plan must be filled out for students with known allergies and/or use an epinephrine auto-injector.  

  • Asthma Plan

    Asthma Plan - An Asthma Management Plan must be filled out for ALL students with asthma.

  • Seizure Management Plan

    Seizure Management Plan -  A Seizure Management Plan must be filled out for students with a known history of seizures.
