The school maintains a clinic for students. When students become ill or injured during the day, they should report it immediately to the teacher and request a pass to the clinic. Clinic workers will contact parents as situations require. For this reason, it is imperative that parents complete a Clinic Card at the beginning of school that includes emergency phone numbers. Clinic workers supervise the administration of medications, take temperatures, and administer emergency first aid when appropriate.
In order for a student to receive any kind of over the counter medication at the clinic, all students MUST HAVE an Administration of Medication Request Form on file with the clinic. A new form is required for each school year. Please fill this out and return it to the clinic. You may also fax this form to 678-957-3108.

Administering over-the-counter medications at school is discouraged. If a student must have medication in order to attend school, the parent/guardian must furnish a written request for the school to supervise the administration of the medication. This request must include the student’s name, the name of the medication, dose and time to be administered. Medication must be kept in the clinic in the original container. If the medication is to be administered for more than one day, a Medication Request Form must be completed. Students may not have over-the-counter medications in their possession and may not share medications of any kind with fellow students.
The clinic will provide Tylenol, Advil, or tums with the Medical Permission Form filled out. The parent/guardian MUST completely fill out this permission form, must circle which of the meds may be administered and signed by the parent/guardian. If you child should come in for this medication repeatedly (more than 3 times) the clinic will ask the parent to provide thier own bottle of the medication and will be kept locked in the clinic for their child.
Taking medications during school hours is discouraged. Parents are asked to arrange medication schedules so that it is not necessary for medication to be taken at school. However, if the treating physician recommends such administration of the medication, then the parent must complete the Administration of Medication Request Form and return it to the school. This form asks for a specific length of time the medication is to be taken and instructions including the dosage and time of administration. Prescription medications must be in the original prescription container and must be housed in the clinic. If medications must be given over a long period of time, then the Administration Request Form must be updated yearly. Students may not have prescription medications in their possession or share these medications with others.
All prescription medications, including Epi-Pens and inhalers must be picked up by the parent/guardian at the end of the school year. Any medication not picked up will be discarded.
** All medication must remain in the clinic with the exception of an inhaler or Epi-Pen which requires a Doctor's signature along with Administration of Medication Request Form signed by the parent.
**Any Medication must be brought in by the Parent/Guardian. All medications must be picked up by the end of the school year by a Parent/Guardian or they will be discarded.
